Liability Release and Waiver for services performed at Moscow Mountain Sport and Physical Therapy and the Reactiv Training Center:

In consideration of being allowed to participate in an exercise program/class at Moscow Mountain Sport & Physical Therapy (MMSPT) or Reactiv Training Center (RTC) or P2P Performance (within RTC) located at 904 East White Ave Moscow, ID 83843/2009 S. Main Street Moscow Idaho 83843.

• I do understand to use these facilities, exercise equipment or any location used by MMSPT/RTC/P2P to conduct a program, I for myself, my dependents, heirs and assigns, do hereby waive, release and forever discharge MMSPT/RTC/P2P, their employees and investors from injuries or damages resulting from my participation in fitness/exercise classes or use of its equipment in its program. 

• I understand and am fully aware that exercise, including use of exercise equipment is a potentially hazardous activity. 

• I/my dependent am voluntarily participating in these activities and using equipment with knowledge of the dangers involved. 

• I hereby agree to expressly assume and accept all risks of injury or death associated with any fitness program at MMSPT/RTC/P2P. 

• I hereby declare myself/ dependent to be physically sound and suffering from no condition that would prevent my participation or use of equipment in an exercise program. 

• I acknowledge that I/my dependent have either had a physical examination and have been given my physician's permission to participate, or I have decided to participate in physical activity and use equipment in an exercise program without the approval of my physician, and do hereby assume all responsibility for such participation.

Photo Release Form -

• I hereby grant MMSPT/RTC/P2P permission to use my likeness in a photograph in any and all of its publications, including website entries and social media platforms, without payment or any other consideration. 

• I understand and agree that these materials will become the property of MMSPT/RTC/P2P. 

• I hereby irrevocably authorize MMSPT/RTC/P2P to edit, alter, copy, exhibit, publish, or distribute photos for purposes of publicizing MMSPT/RTC/P2P’s programs or for any other lawful purpose.

• In addition, I waive the right to inspect or approve the finished product, including written or electronic copy, wherein my likeness appears. Additionally, I waive the right to inspect or approve the finished product, including written or electronic copy, wherein my likeness appears. 

• Additionally, I waive any right to royalties or other compensation arising or related to the photograph.
